Lipedema surgery
Lymph-sparing surgical fat reduction is, for the right patient at the right stage, the most decisive thing we offer. It is the only treatment that removes the diseased lipedematous tissue itself, and it does so with deliberate care for the lymphatic system that lives in and around that tissue. It is medical, not cosmetic — the goals are pain reduction, restored mobility, and a slower disease trajectory.

What is lymph-sparing lipedema surgery?
Lymph-sparing lipedema surgery is a specialized form of liposuction that removes lipedematous fat while preserving the lymphatic vessels that course through it. It uses fine cannulas, carefully placed access points, and tumescent (or water-jet, or laser-assisted) techniques. It is fundamentally different from cosmetic liposuction in three ways:
- Intent. The goal is medical decompression of a diseased tissue, not cosmetic re-shaping. The body responds differently when we treat it that way.
- Technique. Cannula choice, motion, plane of work, and volume targeted are all chosen to spare the lymphatic system. A surgeon doing cosmetic lipo on a lipedema leg can do real harm.
- Staging. The disease is too extensive for one session. Treatment is staged across multiple procedures, prioritizing the regions causing the most pain or function loss first.
Who is a candidate for lipedema surgery?
Not every lipedema patient needs surgery, and not every patient who wants it is a candidate today. We consider patients carefully against several axes:
- Stage. Most surgical candidates are at Stage 2 or beyond; we sometimes offer Stage 1 patients an early intervention to interrupt progression, but it is rare.
- Conservative care faithfully tried. Patients who have done the conservative work and still have pain, function loss, or visible progression are the ones for whom surgery delivers the most.
- Health and risk profile. We screen for cardiovascular risk, anesthesia tolerance, clotting history, BMI/co-morbidities, lymphedema overlay, and the practical capacity to recover well.
- Expectations. Patients who arrive expecting cosmetic perfection are not the patients who do well. Patients who arrive expecting functional improvement and pain reduction are.
What the goal is
We design every surgical plan around three medical outcomes, in this order:
- Reduce pain. Removing the chronically inflamed lipedematous tissue lifts a baseline of pain most patients have lived with for decades.
- Restore mobility. Inner-thigh shelves, knee-overhang tissue, and upper-arm lobules cause real mechanical limitation. Removing them changes how patients walk, dress, sit, and exercise.
- Slow the disease. Decompressing the tissue and offloading the lymphatic system meaningfully changes the trajectory toward Stage 4.
Visible shape improvement does happen, and it matters — but it is a side effect of doing the medical work correctly, not the point.
Important: surgery does not replace conservative care
Every surgical patient continues conservative care — compression, MLD, anti-inflammatory nutrition, lymph-friendly movement — before, during, and after the surgical journey. Lipedema is chronic; the lifelong floor is conservative care, and surgery is layered on top when the tissue itself needs to come out.
